All you have to do is complete two steps:

Visit your Primary Care Physician (PCP) for an annual physical with lab work. Take the 2024 Physician Results Form to your appointment to be completed by your Physician and submit to Wellworks For You.

KNOW YOUR NUMBER ASSESSMENT

The Know Your Number (KYN) is an evidenced-based health risk assessment and education tool, which evaluates your individual risk for six (6) of the most costly, modifiable chronic diseases. Complete the short assessment on the Wellness Portal or Mobile App.

CPM believes the wellbeing of its team members is important. As such, CPM wants to encourage and reward you for proactively taking care of YOU! This is a voluntary benefit, so you do not have to do anything if you don’t wish to, and it will not impact your benefit eligibility or plan coverages.

The Wellworks For You program is designed for employees who are enrolled in the medical plan. Participation is completely voluntary and does not affect your ability to be covered by the plans, or the level of coverage the plans offer.

The Premium Discount (for those who complete and submit their Physician Results Form will be $20.00 per month beginning in 2025.

An annual physical is one way to ensure you’re monitoring your overall health in partnership with a healthcare provider. Those visits are the best place to help identify conditions or concerns at an early stage, before we experience symptoms, to minimize the likelihood of more significant issues.

No. Only the medically-enrolled employee is required to complete and submit the Physician Results Form to complete the program.

The Annual Physical with labwork can take place at any time between October 1, 2023 – September 29, 2024 to receive credit.
If you already completed the annual physical exam and lab work, you can bring the Physician Results Form to your healthcare provider, have them complete the results of the exam and lab work, and sign the form for you to submit it to Wellworks For You.

Many providers will want to meet to go over the lab results. If you have that appointment scheduled, take the form in for that visit and ask the provider to fill in the results and sign the form.
If you do not have another appointment, please call your provider and let them know you need to have the provider complete and sign a short form to earn a Premium Discount through your employer. Most will ask you to email, fax or drop off the form so the provider can complete and sign it.
Once you have the completed form, you can submit it via the Wellworks For You portal or app to complete the program.

To provide everyone covered under the plan with the same opportunity to earn the Premium Discount, we are using the current Wellness Year to allow everyone to complete their lab work and preventative physical exam. Some employees may not have a primary care provider who can see them for an annual preventative physical exam. The wait to schedule a preventive exam can take some time. We’re giving this amount of notice so that everyone (including those without a current doctor) will have time to schedule and exam and complete the requirements if they are interested in earning the Premium Discount.

The Physician Results Form is required to be submitted to Wellworks For You, and it requires your physician’s signature to confirm the accuracy of the results and that they conducted an annual physical exam. If you have the results from the lab work your provider ordered, you can certainly print a copy of that to give to your provider, making their job filling out the form even easier.

The completed Physician Results Form must be received by Wellworks For You no later than September 29, 2024.

All individually identifiable health data is strictly governed by HIPAA, and Wellworks For You – along with all CPM benefit providers comply with these requirements. Your individual healthcare data is never sold or shared with anyone who has not been authorized to receive it. Wellworks For You receives this data solely for the purpose of administering the wellness program and the associated incentives.
In addition, Wellworks For You does provide aggregate summary data (not identified at the individual level) to help identify health issues or common gaps in clinical guidelines that will help CPM as we consider new benefits or approaches to help employees and their family members. As noted, however, no individual health data is ever shared with CPM Holdings.

  • Go to wellworksforyoulogin.com or Wellworks For You mobile app
  • Your account credentials are listed below
    • Username: CPM+First Name+Last Name+ birth year (CPMJohnDoe1985)
    • Password: Birth date in MMDDYYYY format (01021985). This is a temporary password, and once you login you will be prompted to change it. If you have forgotten your password, just select Forgot Password to change it.
  • If enrolled in medical coverage after December 16, 2023
    • Go to wellworksforyoulogin.com or download the Wellworks For You mobile app
    • Click the register link and create an account
    • Enter your company ID: 12919
    • Select Yes to confirm the company name listed is correct
    • Create a Username, Password, and enter the required personal information
    • Select Create Account
    • Accept the terms of the consent form

Visit UMR.com and select “Find a provider”

Type “UnitedHealthcare Choice Plus Network” into the provider network search box

For medical providers, choose view providers. You will be direct to the UHC website to find providers near you

ellworks For You requires at least 7 – 10 business days for processing and participation to be updated in the wellness portal. Submission via wellness portal or mobile app, will results in an immediate confirmation that your form was received. Any other means of submission requires you to log into your wellness portal or mobile app to confirm your form was processed.
